You can find the Witch’s House just West of the town here. There is a poisonous vapor inside so make sure you go in at full health and with additional rations to recover from the poison. The map is resting on a table inside. The entrance to the hut is around the back, there’s a wood board blocking a passage that leads under and into the house. Destroy the wooden board to gain access.
